Documentation to provide to civil surgeon to prove that ONLY vaccinations are required for i-693?

My wife, who entered on a K1 in April 2017, married in June, went to a civil surgeon today to get her vaccinations for the 693.  He balked at filling out the form without "a letter" saying she needed only the vaccinations but not the full physical.  I told him we didn't have any such letter, but that she couldn't have gotten her K1 without the physical.  He sent us away and told us to get "a letter".  

 

Does he really need documentation to fill out the 693 for vaccinations only?  The USCIS website is very clear that if the exam for the visa had been done within a year, the physical is not necessary.  But the website does not say how to document that.  Apparently this CS expects this documentation?

 

Does anybody know how I can satisfy this doctor?  Is there such documentation I can provide?

Find a different CS. If he's being like that I wouldn't want to give him my money. Call around and ask before booking the next appointment to find a CS who will. I think there's a list on here of CS who don't rip you off.

So, just to be clear, nobody is aware of any such documentation I can provide to the civil surgeon, right?

 

I did ask if he did vaccination-only 693s, the receptionist said yes.  We got the the appointment and wait in the exam room and the doctor takes blood pressure and goes to another room to get the shot... literally as the vial and syringe are in his hands, he stops short and asks me if we have documentation to prove that she only needs shots and not the full physical.  I say no, I don't think there is such documentation, he says, "look, I won't charge you for this, but go talk to your lawyer and get a letter".  I ask him "what letter", and he can't be specific.  Just says he gets so many of these 693s rejected that he recommends we get "a letter" so he knows he can do shots only.  So we leave the office without the shots and completely confused as to what "letter" he could possibly want.

Hello. Look at this site. This is the United States Public Health Service CDC Site that certifies this Civil Surgeon you are using. Complain to them if he doesn't follow their guidelines. A medical exam is required too.

A medical examination is mandatory for all refugees coming to the U.S. and all applicants outside the U.S. applying for an immigrant visa. Aliens in the United States who apply for adjustment of their immigration status to that of permanent resident are also required to be medically examined. Aliens applying for nonimmigrant visas (temporary admission) may be required to undergo a medical examination at the discretion of the consular officer overseas or immigration officer at the U.S. port of entry, if there is reason to suspect that an inadmissible health-related condition exists.

I believe the exam has to be done within six months not a year to be valid. See below

NYCruiser - thanks, but your first post doesn't address my issue, and your second post claims a six month window, which is incorrect.  It is one year, according to the 693 instructions.  It's ok not to know, but please try not to confuse things more than they already are.

Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.
