He can also go to the civil surgeon once he gets here and fill out Part 1,2,4, and 7 of form I-693.  It is very easy and should be at reduced charge depending on how many vaccines he still needs.  Some civil surgeons will do it for free if there is no exam and no vaccination. They will give you a sealed envelop to be sent with your AOS application. This way things will go faster and without an RFE.

Part 1 is about the applicant... name, address, numbers, ...
Part 2 is about the interpreter (if any)
Part 4 is about the civil surgeon information (name, address, number)
Part 7 is about the vaccines taken.

 

You have to prove that the vaccines were taken though otherwise the civil surgeon will have to give the missing vaccination.

A lot of doctors will argue with you and tell you that you don't get a copy but insist on it and it also should say 'copy for patient'

somewhere on the form.

Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

Most CS will incorrectly insist you redo the entire medical again and charge a lot to sign off on the vaccs.
If the vaccs were marked complete there is no need to go to a CS. If not then yes as stated.

I agree with that... and they also tell you we have never done it that way and it would not work and so on.  In that case, print out the instructions of the form I-693 and show it to them.  It states clearly that for K1 AOS, if the exam is done within the last 12 months then fill out the four parts stated above.  Then the civil surgeon will have no other way of arguing and they have to do it for you.  I know people who did it that way, and the 4 parts were completed  for them for free (because they needed no exam and no vaccines).

 My fiance called the Dr. and was told they forgot to give it to him. Please be aware (Hamburg) location. So they will be mailing it out to him. Thanks for all the advice.

The DS-3025 is to be sent in with the AOS package, that's why you need it. And it should be marked 'complete'. USCIS should also match your AOS application with the sealed envelope you hand over to the CBP but often they fail to do so.

I didn't had this form , and I didn't send it with my AOS package and I got no RFE asking me to do the medical exam again

Just because you didn't get an RFE doesn't mean that others won't. The same way I didn't get an RFE for not stating how we initially met while most others get RFEs for doing the same thing.
