Aos fee $1070 - This is Correct.

Ead is there a fee? - If filing the I-765 with AOS, there is no extra fee. (Same also if you are filing for AP I-131)

Ead question #16?what to put m confused a6?c9? - You should use C9

I864 iam the petitioner what to check in part 1 #1??? - If you are the petitioner, and filing for your fiancé, you should check 1A

How much total do i need to pay?? - $1,070.00

Good Luck!

It looks like you have everything.

Only thing I am not sure on is the DS-3025 as we did not have that. The medical for my husband was completed in Mexico, but we had to submit the Form I-693 for the Vaccination proofs. Not sure in your situation if that is needed since you have the DS-3025.

Also, I sent a full copies of both passports. Not sure if that was required, or just what I did. I also sent a copy of my birth certificate with the Form I-864.

Otherwise, everything seems to be covered!

If sending through USPS, then yes that is the correct address.

He did medical in the philippines..valid for a year

Look over his DS-3025 to make sure it is complete, otherwise he will need to schedule a trip to a Civil Surgeon to make sure it gets completed. Some DS-3025's are marked incomplete.

For the mailing addresses here are the two addresses. If you use something with signature delivery (fedex, ups, dhl, ect..) you will need to use the Dearborn address. If you use standard USPS send to the PO Box.

For U.S. Postal Service (USPS) deliveries:

USCIS

PO Box 805887

Chicago, IL 60680-4120

For Express mail and courier deliveries:

USCIS

Attn: FBAS

131 South Dearborn - 3rd Floor

Chicago, IL 60603-5517

That's because they are submitting I-485, I-765 (to apply for EAD), and I-131 (to apply for AP), and each of them requires two photos. It will be less if you don't apply for some of those.

He is not applying for I-131 (AP). OP I do think you should. It's a very simple form and it requires just additional copies of the same documents as the EAD. Please file it. It also costs nothing and will allow your spouse to go home without issues in case of an emergency or in case your green card ends up taking a long time to process. There is absolutely no downside to it.

It is 2 passport photos per application. 2 for AOS, 2 for EAD, 2 for AP.

Do include the DS-3025.
