Hi All,

My fiance and I married on 2/18 in Arizona. When I filled out the marriage certificiate, I specifically asked them if we should put her new name (my last name) on the form. They said NO, that it had to be what was on her Passport.

Now I am filling out the SSN (SS-5), the I-486 (AOS) and the I-795 (EAD) forms and using her new name. Is there something else I should be doing to make sure the new name is accepted?

better to apply for a SSN in visa name, then get it changed later when EAD card in hand.

If she changed her name via the marriage then use the new legal name. If not, then she needs to wait until naturalization (when it's simple to do) to change it or you must do the legal name change via your local court house.

The SSN process and the AOS process are 2 separate things.

For the SSN, you should apply in the visa name. When you get the EAD, go back and change it to the married name.

For the AOS, yes you can and should put the new married name on all the forms. It does not matter that your marriage certificate only lists the maiden name, the marriage certificate is still a legal name change document.

Quick update:

We planned on submitting our SSN application with the Visa name; however, when we met with the SS Rep to apply for the card, he said now would be the best time to request the SS Card with the new name. That is what we did. For the AOS, we will do the same, but I will verify with the EAD on how to best submit (Visa name or Married Name). Thanks again.
