Ok that makes SO much more sense now! I thought those were things we had to fill out!

Yes, they took his packet at POE. So then there is nothing more we need to fill out as far as AOS packet goes, right?

As far as the medical issue goes, no, there is nothing else you have to do because his medical info will meet up with you at the AOS.

If in your "sealed packet" at home you happen to find a DS-3025 copy then you can look if it is marked complete or not to determine if he needs

more vaccinations. If he does then you get the shots and have a CS transcribe it to a I-693 which you then add to your AOS packet.

I think you are correct with that statement. This medical thing is so confusing. We did not have to go to civil surgeon for transcription, but my close friend and his wife, did, because of a

"flu shot" she needed. But, both of our wives, came at different times of the year, thus the flu shot.

As relate to the I-693 form the doctor fill that up sign and stamp it and the nurse place it in the seal envelope that was give to me.

AOS from a K-1(family based visa) does not always require a civil surgeon in the US to fill out an I-693. They will just have had a medical exam overseas and do not need to repeat it if they file for AOS within a year of that exam. They will have seen a panel physician and has DS forms which are used by the Dept. of State abroad for the issuance of visas. Those medical forms will have been included in a sealed envelope they hand in when they enter the US using their K-1 visa.

There are different procedures and requirements for adjusting from a K-1 as opposed to other types of visas such as student, employment and tourist visas. Someone adjusting from a K-1 will not necessarily have an interview either, nor do the file an I-130 with the I-485. These differences is why there are two separate AOS forums for adjusting from the different visa types.

OP> If you are sure you had all the required vaccinations at the time of the K-1 medical, then put a note on your cover letter stating that all required vaccinations were complete and the DS-3025 can be found with the other K-1 medical documents turned in at his POE that were sent to the USCIS.

This question seems to come up a lot. More than likely your packet only contains the X-Ray. My husband's only contained the chest x-rays. Often times the clinics in Ghana do not give you a copy of the DS-3025. I was a little worried after reading that some people were given an RFE for not having the form so I went ahead and had the DS-3025 filled out (for my own peace of mind). A civil surgeon will transcribe the vaccines from the vaccine card onto the DS-3025.Go to this thread and find a civil surgeon in your area that will not try to force you to get another medical or charged you as much as you paid for the process to transcribe. http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/ Without seeing a physical copy of your I-693 or without having a DS-3025 you don't know if the doctor checked off vaccinations complete. Especially if all of the vaccines were completed after your interview. It's better to be safe than get a RFE. IMO it's your call if you want t get it transcribed or include a letter.

He should have been given a sealed envelope not to be opened after the medical exam place to hand over to the CO at the interview.

I don't now how your embassy handles things there or if this envelope gets send to the embassy from the medical place.

Ask in your regional forum.

Just to clear up something that could confuse people reading your post:

A panel physician does the K-1 medical overseas through DoS(Dept. of State). The DS-3025 is part of the DoS medical, and the other medical forms are also DS forms. There is no I-693 with the K-1 medical.

A civil surgeon does immigration exams within the US for the USCIS and they are the ones that use an I-693. If you go to a civil surgeon in the US, then you would have your vaccinations transcribed onto an I-693 by them, and not onto a DS-3025.

His visa will be delivered with a sealed envelope that tells you not to open it. All his file documents are in that envelope. When he enters the US using the K-1 visa, he will have to hand over that sealed envelope at his POE.

The copy of the DS-3025 and x-ray CD he received at the medical are his to keep.
