What help do you need to write that? You simply state when you went on vacation, where it was, and that you met your fiance(e) in person for the first time during that vacation. Add in how much time you spent together, and add in places you went together, things you did, meeting their friends and family etc.

The most important proof of meeting in person are your passport stamps, followed by boarding passes and ATM/Debit receipts and/or credit card/bank statements that are from the location of your vacation during the time you state you were there. Supplement those things with some photos together. If you have any in front of landmarks and with friends/family, then use those.

Only one or two sentences maximum.

Example: We met in XXXXCity during the period of September XX to XX, 2014.

Phil (Lockport, near Chicago) and Alla (Lobnya, near Moscow)

As of Dec 7, 2009, now Zero miles apart (literally)!
