Is There Any Form or Sample for Affidavits Sworn OR Affirmed for I-751, Petition to Remove Conditions on Residence

Hi, I will be submitting for my application for petition to remove conditional on my permanent residence in State in few month time. I need some advise on where i can find a sample or is there any form for Affidavits Sworn or Affirmed? I read on USCIS website and they require this when we do the submission to our petition but i couldn't find any sample or form for it.

They are not required if you have plenty of solid evidence to prove your marriage. There are some samples here on VJ in the examples, or you can also do a search and come up with some.

Thanks all for your replied. I will search more regarding this. It was stated in the USCIS instructions for petition to remove condition on residence for evidence of relationship. So i'm not sure is it compulsory to submit as well. Worried that they will delay the application due to this.

Hi, I'm from Singapore and i got married in USA after entering into US with my K1 visa. My green card will be expired in mid Nov this year. I'm currently getting ready to gather some of my paperwork for submission before 90 days which mean i need to submit around mid Aug. I need advise on submission of affidavits sworn to or affirmed by at least 2 people who have know both of us since my conditional residence was granted for evidence of the relationship. Is there any sample or format letter on this that i can follow?

I will be looking forward for your advise.
