Ok here is the thing

I had my madical on 4/23/2015 (received results on 5/4/2015). Visa was issued on 5/18/2015 and visa expiration date is 11/17/2015.

Is the expiration date correct or should I enter to US before 10/22/2015 (six months from the medical date)?

You can enter US between visa's issue date and visa's expiration date. Don't forget to pay immigrant fee of $165 or you won't get a green card via mail at your US address. Yes, you can enter before 10/22/2015. that's fine.

I see why OP is confused. I read somewhere on this forum to have a valid (not expired) medical exam reports at POE. Is it not mandatory?

Yeap, that is why I`m concerned if my flight will be after 10/22 which is 6 months from my medical exam....

Call the Consulate/Embassy or email them with this question. Or go before your medical is over 6 months to be on the safe side.

I was under the impression the date on the Visa was the expiry date, I even heard many people asking this question upon receiving their passports back and they were told the date on the Visa is the expiry date.

I`ve sent an email to the embassy.

I`m asking this because I have to resign my job and I have 4 months resignation period.

You have six months from the date of issue to enter the US. If your medical happens to expire before you enter you need to have the medical re-done and travel before the expiration of the visa itself. Congrats!

Good to know, my Consulate was telling people the wrong thing!!! Imagine arriving and getting turned away?

Not sure how the embassy came up with 11/3. Your visa expires 11/17/2015 while the medical exam expires 10/22/2015.
