When I had my medical examination abroad, the doctor didn't give me a copy of the DS-3025, They did give me a vaccination record card, can I just bring this to the civil surgeon to get the Vaccination Supplement? Has anyone experienced this ? Or did everyone get a copy from the doctor?

I am not sure about your problem.I would make some phone calls to as many "Civil Surgeons"as is possible to hopefully find one that will fill out the supplement.We have her DS-3025 and are still having a problem with getting it filled out.Good Luck Her is a copy of the supplement if it will help.

02/03/05...We pick up Visa!!!!!!!!!!!!!!!!!!

04/15/06...Offical marriage in U.S.A.(Tax Day) ;) ;)

Everything we have done was without the help of an attorney.We have not had any problems thus far and will not,I believe.

Look very carefully at the vacination card. I think you might find tiny numbers that say DS 3025 on it somewhere. The DS 3025 is just a small card that records what you have and what you need. This is turned into an I 693A by a civil surgeon.

I got the vaccination supplement :) And I went there only with my vaccination card record and empty I693 appendix A form. The doctor moved all the vaccination records to the form, and did a blood test to see if I'm immuned to chicken pox or I still need the vaccination for that. This morning I got a call from them saying the form is ready to pick up :dance: now just waiting for the taxes to be done and we can send in the AOS package.

The DS-3025 is the Vaccination Documentation Worksheet filled in by the US Civil Surgeon seen for the K1 (or other) medical examination. It's not the same as a vaccination history card.

From what I have been reading here, I think you only need to send one, which is I693 appendix A (The Vaccination Supplement). Some people sent in DS 3025 instead of that and not get an RFE but others did, so I think it will be safer just to get the I693. And thank you Becky :)

Does anyone know if it has to be a US based civil surgeon? Can't you take this PDF file to the Dr. who did the initial exam in the home country and have him transfer the vacc. from the card to the 693?

Some civil surgeons will transfer from the vaccination card, some will give you different degrees of a hard time. Try contacting Dr. Leonard in Austin, TX to have your information transferred to the I-693A. Her office will call you back if you leave a message.

The doctor's in Germany sometimes forget to provide the applicant with a copy of the DS-3025. Happened to me as well. I called them and had it faxed to my parents, they scanned it and emailed it to me. The civil surgeon I went to in the US told me, that it would have been possible also to transfer the data from my vaccination pass/record.

go for the dr arnold! im pretty sure she can transfer the info form you vacc card to the needed I-693A and shell do that for only $10!!

most civil surgeons i called wanted me to get the whole medical again and pay $150 or more.....
