Hello, I'm doing the K1 change of status. And I'm pulling my hair out. In the I 485 instructions . It says to include medical report, which I have. but if you are a K1 do not include it. Just the vaccination supplement. Which I don't think I have. I got a yellow book. international Certificate of vaccinations of Prophylaxis. And I got a Appendix c Form . Vaccines to complete for adults. And here's the kicker. I got a sealed envelope with tape and red stamps . it says To USCIS Vaccination forms. But I cant open it to see what it is. How do I make copies. Who's susposed to get it. And what if they lose it.

Here's the extra kicker. She don't have a I94 entry paper. She says they didn't give her one. And Social security wanted it too. When I got her Social security card. She's got her passport / visa entry stamp .

My printer is making lines on the copies. You think they will send my app back.

Yep, SSA will want to see a copy of the I-94 (that you print out), as well as including a copy with your AOS paperwork.

Hmmm...lines on the application forms from your printer??? Sounds like an easy way for an adjudicator to send out an RFE.... just a thought....YMMV

I94 records are online now... please go to Google and type in "I94 record retrieval" and it should give you the link where to go and get the record.... If you are adjusting from a K1 visa, the only medical records you need are the vaccinations... please be sure to look up a surgeon around your area (one who is willing to accept the vaccination records you have and just fill out the corresponding form).... good luck!!!

when you are prepping the AOS set,

note you'll have to have a report from a USA Civil Surgeon,

which means you need to find one, make an appointment, and go in.

Bring the scant medical and vaccination stuff that she has, the doctor will transcribe the immunization record from the yellow book.

as to the I-94, ya, sorry, it's electronic, online now. Printers are cheap, get a nice used HP laser from CL or a pawn shop today or tomorrow. Or print it to pdf and bring the file to any fedex/kinkos and print it out.

IF you really have the medical exam folder (this was to be turned in at the POE, fwiw) then a USA Civil Surgeon is allowed to open it, study it, make annotations in it, and reseal it.

We just did our and printed out the 194 online, you also do not need vaccination record because the sealed yellow envelope that was taken at port of entry includes all the medicals and vaccination done before interview. They take it from you and send i to USCIS and its valid for one year. Read instruction below

Already got her a card. SS found another way to get her a card.

OP you have to get married within 90 days of entry. People will suggest that you do the AOS within the I-94 period, but if you have to say a month or two that is fine. Applying within the 90 days protects your status and you do not accrue days of unauthorized stay. But as said in this post you can wait if you have to.

Dieseltu, we just submitted our AOS last month ... it shouldn't be TOO confusing, the forms are quite straightforward and really very similar to the ones for the initial K-1, so no major surprises. (And yes, as others have said -- 90 days to marry, and file for AOS any time after marriage: sooner is better but there is no official deadline.)

It sounds like your biggest confusion is the medical, and from your post, I'm confused too. Are you saying you still have a sealed medical packet from her medical in China? That shouldn't have happened. The sealed medical report was supposed to be given to her embassy, and the whole packet put together in a sealed envelope, and handed over to immigration control when she arrived in the USA. Did your fiancee not hand over the sealed packet at her port of entry / customs?

At any rate, if you have any copy of the DS-3025, then you CAN simply go to a civil surgeon in the USA here, and have it transcribed onto the I-693. Otherwise if you have a complete medical packet that did NOT get submitted to the embassy/USCIS already, then you'd better send it in with your AOS, because they really are supposed to have it on file already. You can bring that sealed packet to a US civil surgeon and they can open and reseal it for you with their signature.

Ok well. 11 hours later. I got the forms filled out and on a CD. I got almost all the copy items now. Marriage cert, passport, ect. I got a I94!! I cant figure out the medical part though. I think I got the medical form copy in the sealed envelope. From China It looks like I have to take these medical forms and her to a USA Civil Surgeon? Who opens it and makes a new one in English and seals it up again. To get a copy and a sealed form to turn in with the AOS application . And one of her shot follow ups is due now . It says 4 to 8 weeks.

Anyone want to reply Go for it.

that sounds right - bring in the sealed medical thingie to a USA Civil Surgeon. He's to do some transcription work on form I-693 (I hope I have that right) and she should not need another medical exam, just some medical transcription work.

Usually, that medical pack that you have is handed over to CBP at the POE, but you've got it, for whatever reason. IMO, it's better that you have it and submit it with the I-485, as USCIS usually not match up the medical packet with any casefile submitted for AOS, when adjusting from a K-1.

Yea, I have a sealed medical packet from China. I do not have a DS-3025. I got a shot book. A shot paper, which states whats in the book. And when her shots are due. A medical examination form with a passport photo. and another one. None say DS-3035 I was reading she is susposed to have a sealed envelope to give to the US civil surgeon. I don't know if this is it. BUT, it says its for USCIS vaccination form , on the front.

Here's a good one. 13 hours later, a cd of forms, papers everywhere, and I asked the missus numerous times over two months what's the envelope for. And she says I don't know. And I just told her we have to find a Dr. She says I know for shots, and to stamp the paper in the sealed envelope!!!

Hi there. I'm not in the process of AOS but I've researched it out the wazzoo so I'll answer you're questions. Please refrain from beating yourself with a base-ball bat.

You do not need to include a med report. You only should include A COPY of this: http://photos.state.gov/libraries/thailand/591452/iv/ds3025.pdf

If you do not have the DS-3025, then you will need to fill out the http://www.uscis.gov/sites/default/files/files/form/i-693.pdf with the help of a Civil Surgeon.

You should be calling USCIS and asking them what you should do with the sealed envelope. As far as I know, that was supposed to be handed to the officials at the airport upon arrival so that they could send it off to USCIS. That sealed envelope is for USCIS only. When you call USCIS, Ask to speak with a Tier 2 Representative.

You should deffinately repair/replace your printer. That is prime reason for them to send you RFE.

Even though the got her SSC, you should keep in mind the I-94 is online. You can reference it online for in the future.
