Is this K1? if K1...complete form I-693 for vaccination record that will enclosed envelop from your DR that assigned for immigration, I-765 for employment authorization, I-485 for permanent resident, marriage certificate original, nso birth certificate both of you, I-94 copies, passport copy, visa copy....Goodluck on your journey!!!

You need to send a copy of the front and back of the I-94, never the original!!

Moldovo doesn't have NSO (this is Philippine specific agency)
