What part of the process is this?

AOS part :) im just wondering if i go to the dokter with the I-693, if that is the right document i need.

I don't understand your OP. You mention packet 4, you mention you're "at the I-864" and asking if you can do your medical. If this was AOS (Adjustment of Status) you would have submitted the I-864 and medical (I-693) when you submitted your AOS packet. There is also no "packet 4" for AOS... so it appears you're actually going through the CR-1 process in your home country? Is this correct?

After looking at your timeline, I believe the AOS you are referring to is the Affidavit of Support. This forum is for AOS meaning Adjustment of Status.

You will have better luck getting correct answers to your questions if you follow this guide: http://www.visajourney.com/content/i130guide1 and the one for the Belgium embassy: http://belgium.usembassy.gov/immigrate2/immigrant-visa-issuance-brussels.html and post your questions in this forum: http://www.visajourney.com/forums/forum/110-ir-1-cr-1-spouse-visa-process-procedures/
