Yes. You didn't enter on an immigrant visa and have your sealed medical files sent to USCIS (like K visas do and why we're able to avoid a new medical). Your medical therefore doesn't carry forward. you will need to see a CS and have the I-693 completed.

Yes, it is a must but you can save money/time by finding a recommended Civil surgeon that will accept your previous tests/result when filling out your forms.

Some CS like to have all tests done by them all over again

The medical has to be done by a certified civil surgeon. You can find a list of them on USCIS website, and use the zip code search function to locate civil surgeons near where you live. The costs vary notably from one location to another, and also according to what you need done, for example in terms of vaccinations. Mine was around $180 with everything included (didn't need any vaccinations), but I've seen people here paying everything between $100 to almost $500. I would suggest picking 2-3 doctors near you and calling them to see what their rates are like.

How do u know if u need vaccination or not?

If you have your child vaccines record (in English) then you don't need. Call your mother to ask if she still has it. If not, then you know the answer. The civil surgeon will ask you for that.

Then you can make a plan for the next appointment. Maybe she will not need any. Maybe the doctor accepts verbal confirmation, or has knowledge of national vaccination requirements in England. Maybe some are not age appropriate. Maybe they will accept a scanned copy of vaccinations. Maybe she will need a single shot that you can get free at the county health clinic.

There are too many variables, and you can't make any plan with no concrete knowledge. Make the appointment with the Civil Surgeon, then you will have something to work with. Go to the appointment and see what they say.

Ok guys, thanks all the time u take guiding me through. I had a list of about 10 CS from the USCIS site, n spoke to then all, the best deal i got was for $100.00 if we would provide the Vaccination record and the cool thing is, they accept copies. We spoke to my wife's mum, she gonna email or fax me the records tomorrow, then we ill set our appointment. I can't wait till that piece is out of our way.

Still gathering all the info we need before sending the package, am so more questions will come, i will keep y'all posted. Thanks a million!!!

Ok we got the Vaccination Records scanned to us. So we gonna set appointment with the Civil Surgeon for this coming Tuesday. They accept scanned copies.

But will USCIS require the original copy on our interview day? if yes, then will get my inlaw mail it to us.

No, USCIS doesn't care about the vaccination records you have from your home country. They want to see the I-693 form, filled out and signed by the civil surgeon. That form will have all the information they need, including the information regarding your vaccinations.
