Hi Lise,

I just sent out my AOS application last Monday.

1. The 4 other passport photos go to the EAD and the AP. So, 2 photos for I-485 / 2 photos for EAD / 2 photos for AP. (But, EAD and AP are optional)

2. There are 2 addressed you can send your AOS to, depends on how you mail it: (This is in the USCIS website)

link : address

For U.S. Postal Service (USPS) deliveries:

USCIS

PO Box 805887

Chicago, IL 60680-4120

For Express mail and courier deliveries:

USCIS

Attn: FBAS

131 South Dearborn - 3rd Floor

Chicago, IL 60603-5517

3. I didn't include the SS-5, but went straight to the Social Security Office.

Hope this helps out!

Thanks, I will attach the other 4 to AP & EAD...I am now confused with the I-693 form, I have my vaccination sheet from my medical and a one page report of my xray...if we had our medical do we fill out section 1 and attach the evidence?

Hope your AOS gets a quick approval!!

Lise

Well, I was reading about the I-693, and there's a debate over it. I myself was (still am) confused about it. Some people say that we don't need it because we already had our medical, though some people got an RFE saying that they need a supplemental I-693 still, I guess meaning that their vaccines are not up to date. I went ahead and got a transcription through a civil surgeon, but I suggest doing more research about it. It cost me $30 for that transcription.

Thanks for you input. I found a link earlier on this thread that had the following info:

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status.

I added a supplement to the I-693 with the above info and commented that I had the medical, where & when. I attached the vaccination and xray report. Hoping for the best! I have the I-485 package completed, double checked (I found mistakes...always good to double check!) I am going to review the I-131 and I-765 in a bit and the package will be sent tomorrow :)

Okay, I must be burnt out from paperwork...what am I missing...I had 6 current passport photos taken, I saw on the 1-485 that they want 2 passport photos, where do the other 4 go? Also I want to clarify, do we mail this package to the Chicago Lockbox?

USCIS

ATTN: FBAS

131 SOUTH DEARBORN, 3RD FLOOR

CHICAGO, IL 60603-5517

The last question for today: is it recommended to add a copy of the SS-5 application?

I hope to send this out Friday,

Thanks for the guidance,

Lise

Hey Lise,

Sorry I'm so late seeing this...the other 4 passport photos are for EAD and AP (2 Each).

And Noo...you don't have to do SS-5 with AOS. SS-5 is actually separate for the Social Security Office. It just so happens I was filling everything out at the same time. I filed my Fiance's SS-5 within 2 weeks of him getting here.

Sorry for the confusion!

And yes - we're sending to chicago lockbox

and the other thing is, im sort of waiting for a rfe, that damn medical, lets see what happen

That Medical seems to be giving lots of people some grief! I found a really sweet Civil Surgeon office here. My fiance didn't have varicella shot done for K1 so he has to do it here.

Another thing is we're moving on October 14th...our wedding is on October 1st. So I'm just gonna save myself some extra grief and file after we move so the AOS can be in our new address. I think all that needs to happen is Mr Loxy file AR-11 to notify USCIS that he moved.

Congratulations to everyone who is married already! Lise I love your photo!

Blake and I will be getting married in around 2 weeks, and I am feeling extremely overwhelmed by the paperwork for my SSN and AOS and all that. MORE forms, MORE stress.. I just want to be done with this! At least now Blake and I can be together for it all :)

It's not too bad! The instructions are more overwhelming than the actual forms.
