hi all

i got into the US on aug 18th. i am reading through the aos guidelines in preparation for our aos filing. i need to know how to find a civil surgeon who can do the I693a for me (tfr of vaccination records) pls advise, thanks!

I cannot stress enough how a civl surgeon will try to make you pay for a whole new medical exam. YOU DON'T NEED IT!!!!!!!!!!!!!!!

I used the services of Dr. L. E. Arnold's office last week. They will ask you to send them 2 signed copies of the I-693 Form (download from USCIS website. Send an additional copy of the I-693 with Part 1 filled out so they have your identifying information. I sent the Vaccination Documentation Worksheet (DS-3025) that my wife received at her K1 physical. The fee is $20. They will send you back a sealed envelope that you can submit with your AOS package and a copy of what is inside the envelope. I received my documents by priority mail and had them the same week I mailed them the forms. I highly recommend them. Please note that they are in Suite 114 vs. Suite 106 as mentioned above.

yeah I just called. Civil surgeons really swindle people out of their money. I called 4 civil surgeons, they all said there was NO WAY possible to just do the supplement. Then I stumbled across that doctor in another topic, and I called. They were so nice and had NO problem doing the supplement. So just call and they will give you all the info!
