    Whether or not it is legal to adjust status depends on the intent at point of entry.
     
    Filing a form I-130 is a clear indication that you want to live in the US. 
     
    Link that to coming to the US and suddenly deciding to AOS ?
     
    USCIS could argue that you made your plans to live in the US known as soon as you filed the I-130.

    the amount of people who said " we dont need EAD, we dont need AP" but later on they need it ASAP and cant get it quick and decide to abandon the whole thing just because they said "we dont need it"
     
    always file any benefits that you can get. better to have it but dont need it. no additional fee anyway to file EAD and AP with AOS

    There's no wording such as "cumulatively" or "in total". IMHO this question should be interpreted literally.
     
    If in doubt, can list both in additional information section to be bullet proof.

    If OP is unable to decide where he last lived before coming to US - I am not sure what to say apart from the obvious - most people know where they were living on a particular date.
     
    The date in question is your last day in UK.

    There is a reason they ask you on the form, to disclose all names you have used.
     
    Supplying evidence of those names is a natural progression.
     
    Why are you loathe to do so ?
     
    The form says "you must file your petition with copies of the legal documents reflecting the name change"
     
     Do you think failing to do that is going to make approval of your application more likely ?
     
    Your birth certificate is proof of citizenship. I assume the name on that document is not the name on your passport ?

    If it was me - I would avoid delay / RFE by supplying as much evidence as possible rather than the "wait and see" approach. 
     
    With an increase in approvals after waiving interviews etc- you stand a better chance supplying too much evidence.

    Call on phone, say "infopass" to automated thing - this gets you to a human. Then just ask them.  I was called back by tier two agent a few hrs later.

    I usually do a final check on friday at 5pm and leave it until Monday mornings.
     
    Guess I will be checking weekends now.

    That is a long wait for a decision !
     
    I was under the impression it was the field office that makes the decision.

    I think the USCIS definition of "actively" is not what you might think it is.

    In my case - no update on I-130 when I-485 showed update for biometrics. It only got updated when interview was scheduled.

    I too was "properly petitioned" for.
     
    I doubt my field of work attracts extra scrutiny. It is very common line of work here - more so than Europe. I was not asked a single question about it at interview.
     
    I doubt they conduct an interview before they have finished their checks / vetting.
