I would just like to say, I am so thankful to everyone who posts here on VJ, it has helped my husband and I a lot. Here are my 2 cents!
 
Background
I recommend booking online through their website or even calling to find a date that works for you if online you can't find a suitable time. I called initially, since I did not know about the online booking process. The secretary is very sweet and she was going to give me Dec 5, because that was the most recent availability but then she said a spot opened up for Dec 3. Therefore it doesn't hurt to call and ask as well for an appointment. Note I booked my appointment as soon as I received my appointment letter which was Nov 9 and thats the earliest opening I got.
 
Before arriving make sure you have: 
Passport Immunization record (yellow card, if u don't have yellow card call ur local municipality health department and pick it up. Go to your family dr to review if u are up to date on vaccines) Case # (Appointment Letter) - email printout will suffice anything that is mentioned on their website debit/cash of approx $360 (you pay separately at each step of the exam i.e., x-ray, blood lab, dr's office)  
The Exam Process
I had my medical exam today in Maple, ON (Vaughan /Richmond Hill) with Dr. Ian. My appointment time was 11:15 am.  In the morning's the parking lot is full (it is free) and i circled around for about 5 mins after i decided to park outside the plaza on side road. The waiting room was already filled and I was there by 10:45 am. The secretary called me up at 11:15am, asked for the items I was required to bring and scanned them. She did not take the passport photos I brought even though I asked about them. Instead she took a photo of me on the office camera and uploaded it. She asked for a payment of $260. She also asked about my intended US address, so make sure you know that. I recommend confirming your phone number, so u can be notified of your package.
 
I entered Dr. Ian's office at 11:45am, and it was quick from there. He is a very nice man. He had a light conversation with me about where I am going in the States, if I have family there. He checked my immunization record, asked about any medication I am on, any major surgeries and then he checked my vitals (Blood pressure and heart). That was it and then it just took time for him to enter all the vaccine data into the computer. He then told me to pick up my package in 1 week. Dr then gave me papers and directed me to the x-ray lab and blood lab which are both on the first floor.
 
By 12:05pm, I went to the blood lab to get that done and the urine sample. That costed around $30. I was called in for blood work at 12:25pm. The nurse drew 1 tiny tube of blood and then I went to give the urine sample. (Note: I asked for you all wondering, because I was curious too; for women on their period, you can still give a urine sample, supposedly Dr's can tell that its mensuration blood, so it doesn't interfere with the results. But perhaps it may be better to book around your cycle anyway if you don't want to deal with that.)
 
At 12:39 pm, I was waiting in the x-ray office (it was not packed, thank God). That costed about $70. I was called to go change in the dressing room. I asked to confirm for a female technician (because I am a practicing Muslim). She took one pose and that was it. I was done for the day at 1pm.

I entered the states on January 3rd and I got my green card in the mail by Jan 25! A lot faster than I was told at customs.

I arrived at Pearson at 3:00pm on Jan 3 2019. My original flight was at 7:40pm. I got their early because I thought that my clearance through Customs & Border Protection (CBP), might take a few hours and I also wanted to see if I can get on an earlier flight. Since I had an IR-1, I did not have to get a photo receipt at the Kiosk before approaching CBP. The line was a little long, not too bad, but slower than usual, perhaps the holidays may have affected that.
Anyways, as I approached, I gave my the officer the Visa side on my passport and he just started asking me normal stuff, that they ask b4 they let u enter. Before answering the first question I told him that this was my first time crossing with an IR-1. He was surprised and then asked me about the packet. I told him it was all electronic now. He then took me to the secondary inspection room.
In the room another officer asked me about the packet and I said the same thing. He asked if they gave me any documents and I showed him the Welcome to USA doc, he asked to borrow it and if I could write my intended address on the back. He was gone for quite a bit, but he didn't seem like he knew what to do, because he also mentioned to me, 'this electronic stuff is all new, I have to figure out what to do', and he asked his colleague and supervisor , "so what do we do exactly," I was amused.  His colleague said that ur just gonna double check the info online and then admit her and then she's done. He then called me over to his booth, and said everything looks good, he explained how the visa is used in travel and how my green card might take 6-8 weeks to be delivered since they are experiencing backlog. After that he said congratulations, your good to go!
Also, I was able to get on an earlier flight

I got the notice of the tracking # and that package was picked up by Canada post on Thursday, Dec 27, 2018 (it sounds like a long time but,  there was the weekend in between as well as Christmas Eve, Christmas Day and Boxing day, which were all weekdays, and offices r closed, so i lost 3 business days) The delivery date kept getting delayed, first it was Dec 28, 2018 and then changed to Dec 31, 2018. During both those days the status updates were very slow. Again, I had weekends  and New Year's Holiday in the way. On Dec 31, 2018, I was getting concerned about my package since I had a flight booked for Jan 3, 2019, from before my interview date. So, I called Canada Post customer service, created a service ticket and talked to the supervisor about my situation. She put the package on high priority got on its case. I was still stressed because the tracking service was not updating and it was at huge Gateway distribution centre since Dec 30, 2018, and with all the back log due to the strike i was concerned on delivery delays. I called back on Jan 2, 2018 to talk to the supervisor again, and I was gonna ask if I can go pick it up myself (Ur still not allowed to), but then she said, I have good news, your package has arrived in your city as of this morning and should be at the post office for pick up at 11am. I was so happy, so much stress during this past week.   
As an overview, 
XpressPost is 2-5 business days Excluding all the interruptions (holidays), my package did arrive in that time frame. It may be beneficial to speak to a supervisor to expedite ur process if u have a flight booked from b4.  
So happy now to be going and seeing my husband. New Year, new chapter! All the best to everyone!

In my opinion, it is best to pay fee after receiving the visa and before entering the US.   However, the fee can be paid either before or after entering the US.  Please be aware that the actual Green Card will not be produced and sent to the new immigrant until the fee is paid.
 
EDIT:  The "A" number will be on the visa itself....it is also known as "Registration number"

Before arriving, I made a binder with plastic sleeves and tabs of what is included in the pre-interview checklist. This was not as necessary for the officer as much it just helped me stay organized. My husband took me to Montreal and we drove up and made a trip out of it. I recommend driving up to the embassy the day before the interview to see where to park the day of and where to stand.
 
The Appointment Day
My appointment was at 8 am. We arrived at 7:04 am. We parked in parked in lot across building for $15/day early bird price. After 8am it is $18 for the whole day. It was hard to find parking if you have an appointment before 9:30 am because the streets need to be cleared from 8am-9:30am for city cleaning.
One family arrived right when we did.  Stood in line till 7:45 am. VJ bloggers said to stand on the side with the yellow columns but that didn't matter because the building security guard asked us to line up across the right side of the building.
 
Once we were allowed in we were asked to go to the back of the line since we didn't have a 7:30am appmt and they wanted to prioritize the early ones. So we told everyone in line, if you have a 7:30am appmt move up. And then everyone was kind enough to let us stand in front of the 8am group since we were there early.
When we went in, there was a lady who greeted us and asked us for our appointment time and our passports. She handed us a laminated document that indicated our call number and what we need to have ready for the officer.  She asked out of the two who was the beneficiary and the petitioner. Then we went to security.
The security guy asked us to place our items in the bin like at the airport. Did not have to take shoes off. You hand in ur phone and keys to the officer and get a ballot to pick it up when you leave the building. We went downstairs and waited like everyone else. Then an officer came and told us we r going up to floor 19 and when we arrive we will sit and wait for our number to be called. Our waiting began, it was 8am.
 
When my number was called the lady just asked for all my documents to verify them and passport photos. Confirmed my Canadian address, phone number and then asked me the petitioners phone number and address. She took my finger prints. And then told me I am good to go and to sit and wait again and the officer will call my number for the interview.
 
The lady officer called my number and I went up. She made me take an oath of honesty on the finger print scanner.  She said you were born in Pakistan I said yes. Questions she asked me:
How did you two meet? Who is the petitioner? What does your husband /petitioner do for work? Where does he live? How often do you get to see each other? What is your highest level of education? Have you ever been convicted of a crime? Does your husband cook or do you cook? (meant to be a trick question) Have you ever been denied entry to the US? How long have you ever stayed in the US? Have you ever stayed in the US longer than 6 months?  
Then all of a sudden she pulled out two sheets and said welcome to the US and that our immigration packet is now electronic so we don't take anything to the border. If you have travel plans and are worried about getting your passport back in time, do mention it to the officer. By 8:50 am, we were done.

They did not even ask for the photocopies of the the original documents. 
They did not ask for a printed interview appointment letter.
They did not ask for CEAC print outs.
They did not ask to see petitioners photocopy.
They DID ask to photocopy his W-2.

If you have travel plans and are worried about getting your passport back in time, do mention it to the officer. By 8:50 am, we were done.
