If you know the date is wrong, why would you even consider submitting it? Get it fixed (as it's obviously their error, you should call them up and have them expedite the processing) and then send everything in.

What your fiancée was told is correct - regular testing in the next 12 months and passing the medical exam afterwards is what will get to the visa issuance - another interview is probably in the future as well. Check with the panel physician for timeline/scheduling so it's all recorded and in her record there.
Well for starters your panel physician is using an old form (exp 03/2020), current one is something along 12/31/2025 which is why there's no COVID on its own but listed as other. It looks complete to me. What USCIS will say is anyone's guess - hopefully RFK Jr stops this COVID nonsense soon and they stop requiring COVID for immigrants.
Try having your new USC spouse to get in touch with his congressman/senator to flag his naturalization (and your category change) to local USCIS office.
Yes, this is fine. You only have to get a first one (HepB) for the form to be considered completed. You don't get seconds of varicella or influenza. Same for MMR - she's done with it.
