There is so much going on your lives but I want to give you some options but the first one you are beyond being able to use but there are others that may come across your post and would like to know what all the options are so forgive me for posting something you didn't know existed and quite possibly something you may have considered.
There are three options.
1. The NOA2 is valid for one year. Meaning that you would get the initial 4 months but when you are near it's expiration you could extend it out another 4 months and another 4 until you reached 1 year. The catch is you have to have the Embassy approve the extension. At one year if you had not interviewed your visa application would be "dead" and you would have to start the process all over again.
2. He can stay up to the day before expiration of the visa. He MUST be on US Soil BEFORE the visa expires or he will be returning back home with a denied entry. So leave with plenty of time and watch for weather delays if you are going to cut it close.
3. Get him to the US as fast as you can after approval, get the marriage completed and then get your AOS package off which will include the I-131 TD (Travel Document). The TD is approved in about 3-4 months after submittal. Once he receives the TD card he can return back home but not for a long extended stay maybe 90 days. Anything more than that he may be deem abandoning the AOS and they will deny the AOS. The rest of the AOS package will take upwards of a year to year and a half.
I'm sorry to hear of his mother's illness. My family (brother and sisters) are dealing with it now and I have beat three times.

If your case status says ready. You should be able to submit DS-160 and pay fee and then I think they will contact you for scheduling an interview. Our embassy is similar. At least that’s what I’m trying to figure out because our previous members didn’t leave us anything to work with. Our portal is outdated. Maybe after you figure it out, I recommend you to update your embassy portal so future applicants don’t get confused like we are😳

Nobody can say with any certainty when the case number will be assigned. It used to take 10 business days but for some it's been more then 6 weeks to 6 months. Just have to have patience.

Feeling really bummed... My fiance was here for a month, his flight just left and I'm in a hotel room alone before I drive back home tomorrow....

Finally after 190 days of waiting I got APPROVED!!!!!!!!!
i just checked for the fun of it and did not expect to see it. I’m so happy the long wait is over.
