You dont need to apply for AP but its wise to not omit it as it's both free and if dont have it you cannot leave and return to the US until you get your GC which can take a few years.

Each form is it's own separate submission and requires separate documents as listed on each form instructions.
Each form packet should be assembled separately and clipped together as it's own separate packet (with the exclusion of the I864 as it is part of the I485 submission) and then mailed in one envelope.
Each form is adjudicated by a different officer and is a separate process so you are required to submit the documents as outlined for the individual form on that form's instructions.
I suggest a cover letter for each form as the moment the package arrives, the I765 (and I131) packet(s) will be separated from the I485 packet and will no longer have a table of contents on that packet outlining what that packet contains.
Basically almost everything is in triplicate.
3x (2x 2" by 2") passport photos, so 6 (2 for I485, 2 for I765, 2 for I131).
3x Marriage Certificates (1 for I485, 1 for I765 and 1 for I131).
...and so on. The instructions for each form does out line what is required for each form.

Thanks - this is what I ended up doing!
For more details for those who are interested: I e-mailed Messageries Speedo Monday morning and they e-mailed me back and told me to call. I called and spoke to Sylvain (ask for him!) and he was very helpful. I told him my package needed to be picked up Tuesday (as per the e-mail I received from the Consulate giving me that option) between 10am and 12pm. He needed an authorization letter, which I sent him as well as the e-mail the consulate sent me (I had to wait until I had access to a printer/scanner so I could sign so the office was closed by the time I was able to send the documents. Sylvain called me back Tuesday right in the morning to confirm delivery and so I could pay). I got my package today, Wednesday, before 6pm. They sent it through UPS for next day shipping. Everything is intact and the package that says "DO NOT OPEN" is not opened! I paid 50.75$ total and it saved me from returning to Montreal, staying there for a few extra days, and worrying about the darn Canada Post strike. It's definitely worth it! My interview was last Wednesday and I have my visa in hand exactly a week later. Such an easy process. :DD

Hi everyone,
Here is how i got my passport back without making a trip to Montreal.
I had already seen my status changed to issued online. I emailed montreal embassy and they sent me the generic email.
I contacted a messenger company in Montreal (the one i used was called messageries speedo though i am pretty sure there are other courier companies as well).
I emailed them the pickup email as well as a signed authorization letter (not notarized though) to pick up my passport on my behalf from the embassy.
Their messenger went out on tuesday and the embassy gave them my passport. they sent it to me same day through UPS and i received it today in Halifax in the afternoon.
This whole process was less than 2 days and cost me a total of 55 dollars.

Yup.
They will issue the appropriate (restricted) card, noting that DHS authorization (an EAD) is needed to work. Once you get a green card, you can get a new (unrestricted) card.

You would just apply for a replacement card instead of a new one. The number should be the same.
Same documents.

Number is yours for life. Just go to the office and say you need a new card. Fill out forms and you’ll get it in the mail two weeks later.

Yes I'm sure. Proof of domicile comes up more when the USC is or was living in Canada when filling. As all his info right from the start has been a US address they normally tend to not ask for more info. Take a few copy of things just to help ease your mind.

you need a physical address for the packet to hand in at POE
he can go chnage the address on driver's licence

They didn't ask for proof of domicile at my interview, but I had just once piece of "evidence," his lease...I think it's a pretty low-key thing if the person has been living in the US their whole life.

The Privacy Act police certificate is not a certified document, won't have the fingerprints or a photo on it, nor will it have any embossing either. That's the one that is required for the interview. There is another police check that is certified, with fingerprints and photo on it. A few other people got the regular fingerprint check done, saying the reason is for visa/immigration and that one is certified, with fingerprints and photo on it. Some people are bringing all three with them as a just in case kinda thing.
