Hey guys ..

I am preparing for my AOS documents and i have a question regarding my Medical Supplement form 693 A .. Is it really required to see a civil surgeon for this matter? I have a friend who came her in Georgia and she told me that she did not see any surgeon when she passed her papers for AOS but she just included her medical vaccination sheet from SLEC Manila and she got her GC and all...This confuses me please help.. Thanks..

I am not sure... we also just included the vaccination sheet we got from the medical Rich had done in London. I have also read in other threads about people not getting RFE's and we are hoping we don't get one.

01/29/2007 NOA1!

02/26/2007 Became officially engaged in Rome:)

05/03/2007 NOA2!!!

07/20/2007 Visa Interview in London! Approved!!

10/04/2007 Small private wedding!

04/19/2008 Big fancy shmancy wedding!

AOS Timeline:

10/18/2007 Sent entire AOS, EAD, AP package to Chicago

10/19/2007 Package received in Chicago- signed for by V Bustamante

10/25/2007 NOA1 for AOS, EAD, and AP.

10/26/2007 Check cashed.

11/28/2007 Biometrics appointment in Denver.

12/17/2007 EAD card production ordered!

12/27/2007 EAD card arrives.

12/31/2007 AP approved.

01/07/2008 AP received.

02/01/2008 AOS interview in Denver. APPROVED!

02/11/2008 Green Card received in mail!

Done until July 09' !

Technically you are supposed to go to a Civil Surgeon and have them transfer your vaccination information from form DS3025(the medical vaccination form from your home country medical exam before K1 visa interview) to the I693A.

In reality there are many people that just do not do that and mail in the DS3025 vaccination sheet you are talking about to AOS and they get approved no problems without any RFE for it.

BUT there seems to be no guarantee you will not get a RFE for it or not, because USCIS apparently sometimes catches it and sometimes ignores it or lets it pass, so hard to say. It is up to you if you want to take the risk or chance, the odds appear to be in your favor, worst thing that can happen is that you have to go get I693A after you apply when they send you a RFE, it is just your time is all, so it depends on you.

If you decide to get I693A you need to do a search on this forum under I693A fees or DR L.E. Arnold out of Austin, TX. For he only charges $10 to do this and will do it all by mail and within about 7 - 10 days you will have your I693A form to mail in with AOS application. Sorry cannot remember his number right now but it is already posted out here somewhere.

I sent in my worksheet DS-3025 only. I received a RFE for it at interview in Atlanta. I sent the worksheet to Dr Arnold's office and returned that to the Atlanta DO. All in all, it took about an extra 5-6 weeks from my interview to get my GC.

I just included my vaccination worksheet DS3025 with the application and planned to bring the I-693a Vaccination Supplement with me to my interview. If I had had an RFE I could have sent the completed supplement back by return mail. My file was transferred to California and my application was eventually approved without an interview. I still have the I-683 A Vaccination Supplement here in my personal file. So, it seems to be an inconsistent situation.

Include the I-693A with the AOS if you don't want to risk an RFE, or if the DS3025 shows that the vaccinations were complete at the medical, you can include that with the AOS application with the knowledge that may require the I-693A at the interview or on an RFE, or may not require it at all. The thing is, you just can't guarantee what will happen.

If you choose only to submit the DS-3025 from St Lukes you may or may not get an RFE. I'd still bring a I-693 vaccination supplement to the interview to be safe. with my daughter. her vacinnations were not completed on the I-693 vaccination supplement. we brought her vaccination record to the interview showing they were completed, but we were required to submit another vaccination supplement.

also, the TD vaccination is usually not available at St Lukes. if you haven't had one , that may be noticed missing on your DS-3025.

im still really fing pissed i never got a copy of my DS-3025.. I mean if i would've known i needed I would've asked..

so i had to go pay $120 today to get a fing chickenpox shot that i dont need so a surgeon will sign the 693a form

We only submitted the vaccination checklist. We never got an RFE, and they didn't ask for anything else at the interview and we were approved.

make sure you call them and ask them what information they need form you. i strongly recommend that doc since hes a registered civil surgeon and just charges $10 for the supplement.

You can get the needed vaccinations at the health department. Just submit the vaccination records along with the DS-3025 to the civil surgeon. Cheryl got the Td shot at the health department, and we had Dr Arnold fill out the I-693A vaccination supplement. Worked ok for us.

I just saw a post where the GC was issued without the supplemental but have seem a lot more RFEs because it was missing. I am a 'better safe than sorry' guy so we did it.
