hello! ive tried to look around for answers but figured i would try making my own post. we recently just recieved a request for additional information regarding our K1, and just get told we are "missing page 12" on our form. on this page, however, we didn't fill this one out and sent it in blank since we assumed it was simply a form for information we wish to add if we have anything extra, but we didn't feel the need to do so.

im unsure of what to do, should we simply send it in blank again despite thats what we did previously? or is there anything specific we should add? such as writing N/A on everything and sign/date the bottom of page 12?

also, regarding the request for initial evidence, it says to return the requested information and all supporting documents. does this mean i need to refile the form and reprint my packet? or is it simply referring to the two letters i received, as well as a cover page and page 12?

i would love to hear advice, or what other people have written, especially if you didnt have anything else to add. im feeling very anxious and worried i will do it wrong.. thank you, and i hope to get some answers!

Your success throughout this process relies on your ability to read carefully, interpret literally, and answer completely & honestly.

Looks like they simply want you to print page 12 and complete the information that is asked. If it doesn’t apply to you then you should respond with NA. Then you are to sign and date page 12. The only items you need to return are the cover sheet, the missing information sheet, and page 12. Looks like There are also questions you need to answer on the cover sheet. I haven’t received this particular RFE, but I’m waiting for one. So this is just my interpretation of what I’m reading and not from experience. I hope this helps.

Always read instructions. If the question doesn't pertain than write n/a. Always include every page of forms. You will have several other forms to complete throughout this process. As others have said you need to become an A student during the whole time.

What is the "Edition Date" on the form you filed?  The 2022 edition has a different page 12 for a preparer to sign.  If you filed too late to use that edition, that's a separate issue.  I'm sure the other poster with the same notation filed the older (2022) version.  In both versions the top of the last page autofils when you use an adobe product to complete it.  If the information in the top left corner is missing, that's what you complete.  Page 12 was usually not applicable when using the 2022 version, but IS applicable when using the January 2025 version of the form.  Details matter.

Noting your response in the other thread, I did not say denied.  I said "rejected".  It's not the same thing.  In a rejected filing, the whole package is returned.  That is not the case for you.  I now doubt it was for the June poster either.  My response assumed the June poster used the 2022 version of the form where page 12 is not the last page.
