Welcome to the forums.

One can avoid mistakes if they:

1.  Become an "A" student of the process.

2.  Become an "A" student of the forms which are required.

Not submitting all the required evidence with the I129F is fairly common.  As to the K1 phase, not proving the relationship is real is another common reason.

What are some of the common  mistales/ reasons that K-1 applications get rejected for? And how can one avoid those mistakes?

Inadequate time spent together in person.  Ineligibility (already married, etc).  Major red flags like big age difference.

The U.S. does not legally discriminate based on age for K-1 visas, though a significant age gap can trigger closer scrutiny to ensure the relationship is genuine

.The official eligibility rules only require both parties to be legally free to marry and for the non-U.S. citizen to be at least 18 years old.

We all know they don't say  age as they can't but too big an age difference triggers CO to look for a reason to deny

And for the two to have met in person in the two years prior to filing a petition.
