They are missing page 12 of your petition. So you need to complete it, sign and date it anywhere not in an information field and then include that page when returning your RFE response to USCIS. 

 

It is worded as if they attached a copy of page 12 for you to complete. If they didn't, print it off and complete it.

Thank you so much they did not and that was confusing to me. I will sign at the bottom of the page. Do I need to send a cover letter?

I was told to write my name on the top of the paper, not just the boxes asking for my name. Should I?

The ref just tells me what is above, but I think I might be panicking over the wording and overthinking too much because I want to get it right

make sure to thoroughly read the RFE.

 

but it would say something like "sign/initial and date next to each box you fill out". but if it only says to sign and date, you should be fine to do that in the bottom corner of the page

 

page 12 is just the additional information page, so signing in the bottom corner is where it should go.
