Name change

My wife was divorced and she kept using her ex husband last name. 

She has this name on all the documents we are going to submit.

The only document that still has her old name is her birth certificate. 

We don’t know what to put in the section where it asks for other names that have been used.

Also do we need a proof of name change? And if we do what would it be? Because she didn’t change her name through a court. She changed it through a social security office.

That’s what I figured too. But she said she doesn’t have that marriage certificate anymore and there’s no way to get a new one

which country did she get married in? 
 

Does she have her divorce paperwork?

Which state/county did she get married in? 

When did she get married? We can double check to see if it is impossible to get the marriage certificate... 

She should be able to go to the county webpage and request copies of her marriage certificate.

Giving u examles

• Marriage records: 1847–1949 (all boroughs, but beginning earlier for Manhattan) then the state of NY starting keeping them in Albany
• Many USC's are looking up ancestry records to learn about grandparents,  great grandparents 
• a marriage record from 2011 would be easy to do just look into state info to find out if the state or county keeps the info and pay a small fee to get the record of marriage
• she knows where she was married   /she can get this info

Correct! Marriage certificate is proof of name change!
