We got an RFE for form I-693,  report of medical examination and vaccination record. The letter notes, "All Forms I-693 signed by civil surgeon on or after Oct 5, 2022, must use the 07/19/2022 edition of Form I-693."

 

I completed my medical examination in May 2022, and the civil surgeon signed the 09/13/2021 version of the form. Should I redo my medical examination or would the old one still be OK because it was signed prior to the release of the new edition?

You have to do the new form. No exceptions. 

Immigration journey is not: fast, for the faint at heart, easy, cheap, for the impatient nor right away. If more than 50% of this applies to you, best get off the bus.

I am so surprised that Premier Bintaro used old forms.  Their charge for medical examination is certainly not cheap like in other countries and given that they are the only hospital that is approved by the US Embassy in Jakarta you would think they are up to date in all their forms required by the Embassy there.
