On the I-130 there is a place in the evidence that asks to show Proof of legal name change. We did the Utah Online Wedding the other day and there isn't anything in the marriage certificate that specifically states her name is changing. Also since we did the Utah wedding so we have to process the ROM before she can legally change her name in the Philippines. Once the ROM is done she is free to change her name in accordance to Philippine law with the fathers last name as her middle and take my last name.

 

The simple question is do we need to show anything else beside the marriage cert to show Proof of legal name change? I am guessing I just add the marriage cert to this section as well.

No need to put anything in the name change section of the I-130.  USCIS does not expect you to submit the same marriage certificate twice with the I-130.

Note that your answers are for the I-130. To get the visa and eventual green card in the married name, she'll need to follow the Philippines process for getting a new passport in that name.
