On USCIS website it says what mailing address to send your I824 to based on your original processing center. Just Google it and click on the USCIS website answer that pops up

Hello. I am in the same scenario as you were for my parents visa. i did not to fill out question 62 to mention Islamabad Punjab, Pakistan. Now they are asking for I-824 to send to NVC. Can you please tell on what documents did you attach with your I-824? I am only suppose to send the notice of action and form or should I attach other documents such as thier passport copies with it aswell? 
 

Also, Did you file the form I-824 on your behalf as the petitioner or your husband as the applicant ?
