Most of tbe time it is due to a clerical error by the uscis,  such as a missing stamp.  This us usually cleared up quickly

YMMV

The letter of approval came!
Then came another letter saying that they were reconsidering the decision.

We are waiting for the new letter.

They sent the following information

On, March 25,2021, we reopened your Form i-129f. Please follow the instructions in the notice. If you do no t receive your reopening notice by april 24,2021 please go to uscis.

Is it normal for them to approve and then go back for the decision?

It will be a year in May and we are concerned

It is not typical for the approval of a petition to be withdrawn for reconsideration.  It could be that further information that impacts the eligibility has surfaced.

But your cut/paste/translate looks a bit wonky, and so the actual content of the letter from USCIS is not clear.
