Hi Everyone,

Its been awhile since I have been on here because I filed my forms to remove conditions back in November of 2019.  Received a letter shortly after saying they were extending my green card because of a back log in the system.

Fast forward to end of march I  got a letter in the mail saying they are requesting evidence.

 

Issues with my application were as follows:

1.Income tax return only includes first page of form 1040

2. Address on auto insurance is different than listed on petition

3. Auto purchase contract is only a partial copy

4. Form 1095-B is not sufficient evidence to demonstrate joint health insurance.

 

So my questions are as follows:

1. when submitting tax returns do we just print out the 1040 and submit all pages?  is there something else I can obtain from the IRS that shows it was filed?

2. Auto Insurance is self explanatory as I will just update the address and resend that.

3. We will resubmit the Car Loan Application with all pages

4. What forms can we submit for Health Insurance?  I am covered under my wife's insurance through work as she works for a city government. 

5. our mailing address has always been a PO Box, and we in the last 18 months bought a house.  Should I file a AR11 form to change my address and submit everything with the new house address on it for ease of things?

 

Any help would be great.

1. You can request tax transcripts from the IRS, I guess it's like a summary of your taxes so it's less pages but it has all the information.

4. There must be a portal where your wife can print a summary of her benefits that lists you as a beneficiary. That's what we did. You could also include copies of your insurance cards if they have any relevant information.

You can request your IRS tax returns from the website- they will mail them to you. For health insurance- I imagine you have an account on the insurance website ? If so print out the page where it shows both of you are covered. You could also ask Human Resources at her job for guidance on showing proof of insurance and third photocopy both front and back of both of your insurance cards.

1) either the entire tax return including w2s, all of the 1040, any schedules etc OR a tax return transcript from the IRS (preferable.)

2&3) can also include joint car insurance and registration.  Driver's licenses with your address and both names

4) my husband's insurance is weird snd none of the cards show my name so I printed a copy of the policy showing my name and that it is self+1

5) Yes you could do this. Obviously you are still checking your PO box and should continue to do so but i believe you need to update your physical address within 10 days as well not just a mailing address.
