Hello!

 

I've been doing some research on AOS prior to filing.

 

Can someone clarify what this means on the I-693 form?

"I authorize the release of any information from any and all of my records that USCIS may need to determine my eligibility for the immigration benefit I seek."

If you are adjusting from K-1 you do not to complete any medical examinations if your overseas medical exam (the one for the K-1) was less than 1 year ago from date of filing for adjustment.

 

All you need is proof of your vaccinations, usually a Form DS-3025.

Ds3025 is not required to be submitted,  however many people do

So when I go for my K1 medical should I ask them for that form so I have it already confirming my vaccinations?

Yeah that wouldn't be a bad idea.

 

And as the post before mentioned, it is not REQUIRED to submit DS 3025, but a lot do. Because USCIS will go back to your medical file when you file for AOS, and the vaccination records should be there.

 

So to answer your question; You only need to complete the medical exam once for your K-1 Visa, and the same exam results will be sufficient for AOS if you file for it within 1 year of your medical exam. And it is not a bad idea to ask for a copy of DS 3025 from your doctor although it is not required to submit it with AOS packet per se.

 It is routine for you to get form DS-3025 from the clinic in London showing the records of shots you brought to the appointment. To be ready for AOS without further need to see a civil surgeon  in the US (and form I-693) you have to be signed off by the doctor at the visa medical as complete. Make sure you know the requirements for adult shots and get them before your medical exam. The nurse at the visa medical clinic may  tell you that you can get the later. You can but the you won't be signed off for AOS and will have to pay another doctor in the US to sign off on form I-693.

If you read the I693 instructions within the first few lines you are told to read the exceptions.

K1s are included in the exceptions.

Attempting to fill out a USCIS form without reading the instructions is not advisable.

If your DS-3025 was marked complete then you are good: https://www.uscis.gov/policy-manual/volume-8-part-b-chapter-3 "K or V Nonimmigrants Applying for Adjustment"
