NOTE: Petitions are being denied more, not so much the visas. The article talks about petitions but says visas in the title.

 

The cap of H-1B visas  is still quickly and easily reached per year. No fewer visas are being issued - they're just being issued to different people with stronger cases.

In other words, unless the cap ceiling is lifted, and with an ever-increasing increase in petitions, the denial rate will always increase.

while above is accurate for the capped petitions i.e ~65K a year if i remember correctly, I think because of the tighter scrutiny there is a higher rate of denials for the renewal petitions as well which technically does result in lesser number of h1bs overall since those are not subject to cap

True, we'd have to have numbers on how often renewals are being denied. The majority one would think are new petitions since they have not yet been scrutinized.
