expedite Santo Domingo - Medical Expedite

Has anyone here in the recent months have  had any experience with expedite process at the Santo Domingo embassy? 
 

-how does it work once its been approved? 
-wait time for the embassy to initially get back to you on next steps.

-what if there aren't any appointments available, what to do next?

Its been approved and now is ready in Ceac and at the US embassy.
