Help with Indian renunciation form

It has been so long, I honestly forgot but went back to start of the post and at some point, I had updated what I finally did.

here it is for your reference:

Basically, in first page only put first and last name, and then on next page, put the proper middle name in the middle name section, that's it.

Thanks @Prads ! I did the same, but the renunciation certificate doesn't print the middle name. Did it print middle name for you in the renunciation certificate?

Only way for my middle name to show up in renunciation certificate is if I add that to my first name.

I am planning to use the pdf filler for renunciation certificate and send it.
