Thank you so much for the admin that pinned the info about the medical exam.... I693 and DS3025... After reading your info I do have a question about the immunizations.... Are the only immunizations you listed the only required immunizations? I think these are what was listed on the pinned post: thanks in advance

 

1. Shots marked with a date (at least one of a series)

  • MMR (two are required but if you have "insufficient time interval" Code B in the last column, then you get a waiver)
  • Td or Tdap or DT or DTP or DtaP (One date must be no longer than 10 years ago)
  • Varicella (Either a check by "Vaccine" with a date, OR a check by "Varicella History" if you had chickenpox. History gets you a waiver)
  • Influenza (during flu season)** see note below because you may be okay without it. If it is not flu season, the waiver in the last column is Code F.
  • Everything else marked "not age appropriate" Code A, insufficient time interval B, not flu season F, contraindicated C...things that allow a waiver.

Feb. 14th Interview (2019) APPROVED!!!!!!

Those are the ones that were required in my case: 30 year old. I had to get MMR and Tdap (I believe Td is only accepted as a booster if you had Tdap previously).

It depends on your country of origin (as some countries have other prevalent diseases) and it depends on your age.

Here is the technical instructions for Panel Physicians offering vaccinations for immigration as listed by the CDC. The table shows required vaccinations and what is required by age. The panel physician will determine your need in addition to any you get ahead of time at the time of your visit. In addition, some vaccines can be waived by the panel physician.

Or look at the bottom of the ds3025 and see if was marked "complete" be the panel physician

The OP might feel more comfortable knowing that the actual vaccination section was filled out properly too though as their question was vaccination based. That's why I suggested.

I have been trying to upload the pic of the DS3025  for 30min and it wont let me upload it :( however the doctor did Mark that immigrant visa applicant completed vaccination requirements

Then I wouldnt worry about much else.

Its us6ally the case more that its marked incomplete when it is complete rather than the other way around. Sounds like he knew what he was doing.
