I filled out the little section on the application about how we met. So since I am already including photos of us, tickets/boarding passes, and receipts, that can be used for the personal statement as well?

Personal statement is 3 to 4 sentences.  Evidence addresses the meeting between the two of you within two years of filling. Go up to the blue ribbon and select guides and look for 129f application.  Lots of pointers and examples of what is required.

No you don't have to use the envelope they sent you. If you want to you can tape the little envelope on the package you plan on sending in, with the address showing.

On their website, it states I need to list her name and my name as well as the date. The RFE says it needs dates and locations. should I still put our names? I have 9 photos per page, so I need to write our names 9 times per page?

I went to meet her February this year.
