First of all, I don't think it's an extra toe, I believe it's just extra skin from the side and bottom of her foot that has been pressed out to the side from the way her weight is pushing on her foot.

Secondly, yes, I guess maybe her feet aren't the prettiest, but then she was brought up extremely poor and probably was forced to wear shoes that were too small all the time...

Eta: There's a lot of people who are polydactyl, and it doesn't change the people they are one bit. Halle Berry comes to mind. Now she's a beautiful woman, isn't she?

Aww well first let me say, when i was 14, I was diagnosed with tailors bunions on both feet. Basically, my last toe bones were growing strangely, on an angle, and were pushing through the skin outward, and I looked like I was growing a 6th toe I dunno if that's what this is, but it was a horribly painful condition, which I required surgery to fix. I was told it is a genetic thing mostly in women that normally doesn't happen until you are muuuuuuuch older. Now my mother and grandmother both have the same thing Good job I got mine over and done with at 14! lol and I have a nice shiny set of titanium screws as foot accessories

Polydactyly is actually fairly common but most people tend to have the extra digit removed when they are children. Now, if Oprah was poor as a child she probably wouldn't have been able to afford it. Perhaps she is lucky and it doesn't give her any discomfort (main problems are finding shoes to fit) hence she still has it now, as a adult.

If it isn't photoshopped then I don't think it is skin. If you look above the toe you can see the metatarsal bone line of the foot. Also, a photoshopped pic would tend to just be a repeated toe, this one looks like it isn't fully formed (most extra digits aren't) and it seems to not have a proper toe nail.
