Do i need to send the NOA2 original form or will a copy suffice?

Is it required to fill out and send form i-693? She had her medical exam in may this year.

Does my wife need to send her vaccination records with the application?

what are the supporting documents for Forms i-864, i-765 and i-131?

Addition: All supporting documents are found in the form instructions. I think You'd be better off reading as much as you can..since asking here might mean someone might miss telling you something..

Is it required to fill out and send form i-693? She had her medical exam in may this year. Your wife will be required to see a Civil Surgeon who will complete the form and hand it to her in a stamped, sealed envelope. The sealed envelope must be submitted with the application.

Does my wife need to send her vaccination records with the application? Although not required, my husband sent a copy of the DS-2035 he received at his medical. If your wife has this form she should take it to the appointment with the Civil Surgeon, who should transfer the information to the I-693. If you don't have this form, the Civil Surgeon may insist on administering all required immunizations at an additional charge.

My apologies. My husband had all of his immunizations except his flu shot, so he was required to get that from the Civil Surgeon. I agree with Suss&Camm that if the DS-2035 has all the required info and it's within a year of the medical exam, you don't need the i-693. But I had seen posts on VJ from people who had met the requirements, did not send the I-693 with the AOS package, and were still sent an RFE for the I-693, so you may find that you can get by without sending it. As usual, with USCIS you never know how the individual reviewing your paperwork will react to what you send.

But I had seen posts on VJ from people who had met the requirements, did not send the I-693 with the AOS package, and were still sent an RFE for the I-693, so you may find that you can get by without sending it. As usual, with USCIS you never know how the individual reviewing your paperwork will react to what you send.

This is most likely not due to individual reviewing.. (although at many points pphheeewww! yes that is truly the case :) )

IF the overseas doctor didn't correctly fill out the DS-2035 OR if the USCIS lost that record... then you'll need to, and my guess is that this would be the causes for RFE's, more so than individual interpretations.

So as I see it.. your options are to know that you had your flu shots..hope that the doctor filled in the DS-2035 correctly (unless you know, if you have a copy), and that the USCIS don't lose the record... OR spend the money to go to a civil surgeon and do the i-693... Depending on insurance and cost and what your personal situation is, I'm sure you will come to a decision that suits you OP :):yes:

You are still required to have the I-693 filled out with vaccinations transcribed. Read through the AOS forum posts. Someone just got an RFE for it. All the guides state it is needed.

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

It does not say what you say.. and neither does the form instructions... do you have a link for the person that got an RFE?

I too was told i-693 wasn't needed when adjusting from a K-1 if done within a year from the medical. But I think the DS-3025 has to have the box ticked where it says: "US vaccination requirements complete" and something about "blanket waiver"? You might get an RFE otherwise? I could be very wrong though, haven't even AOSed yet.

Yes I got a copy in the mail the same day the embassy received the original. :thumbs: Did you get yours?

It is ticked, the doctor told me I wouldn't have any issues in the US if he ticked that box, I hope he's right! I didn't get a flu shot as it wasn't flu season, someone else in here told me that wouldn't be an issue even if I file for AOS in the middle of winter - as long as it wasn't flu season when I did my medical I'll be fine. Would've sucked having to go to a civil surgeon and have to fill out the i-693 for that one shot only.

No.. I didn't request one.. I wonder if I can request now.. but you were at the clinic in Stockholm? If so I should be alright.. but it would be nice to actually see it hahaha :)
