AOS for K1 Questions about medical and name change

I just want to make sure I am clear about the medical examination and name change.

My husband took his medical examination at the embassy but they did not give him any shots therefore he must get his here, right? Anyways, do we get those done before sending in our AOS package and if so how? If not, do we send in the package and wait to get the notification about going to see the civil surgeon??

Also, my husband and I have changed our names since we married. We both had two second last names and now just have his and have of course kept our first and middle names. Now that we are filing our the AOS do we fill out everything with the new information of our names?

for example:

Before we filled out our application with the names Frida S. S. I And his Ricardo A. E. C. each of the initials representing names. Now I am just Frida S. E. (adopting his last name) and he is Ricardo A. E. dropping his second last name.

Please, any help and guidance will be greatly appreciated. Thanks guys!

As for name change, if it is legal in the state you married for you to drop a name through marriage, then yes, you can just fill out the AOS forms with the new names. If is not legal to do a name change in that way in your state through marriage, then you will need a court-ordered name change first.
