UPDATE: Went to see the nurse at my surgery. They found my MMR records. Turns out I had the MMR as a child and then had a Measles booster and then a Rubella booster but not the mumps! She gave me the mumps one today but it's now listed on my records as an MMR booster rather than just mumps. Hopefully that is enough to satisfy the requirements for that one?

I had a Pertussis jab as a child... so that means that I just needed Diphtheria and Tetanus, correct? I had the combined D and T booster today as well.

Hopefully that is me covered now, jab-wise!

We're in AOS he** right now thanks to Knightsbridge. After waiting 10 months of waiting we got a RFE for immunizations. All of my husband's immunizations were completed with his medical in January 2014 to include the seasonal flu vaccination and varicella history. Unfortunately, the person who filled out his vaccination record recorded his tetanus as January 2016 instead of 2014. We're now out an additional $450 to correct this. After contacting 6 civil surgeons in our area, none would just give a new tetanus and update the immunization record. They all required a whole new medical.

You can now add checking correct dates to the list.

Need some assistance: Here is his completed DS-3025.

Shot dates are correctly listed. VH is correctly ticked. However, there are no ticks where I thought I should expect to see some i.e; TD (his date is written down and marked with a B but has no TICK) the MMR has the correct dates but has no mark code or TICK). Does he need to get on the phone and raise some hell?

If it is indeed correct, I hope that others can use this as an example of how it should be correctly filled out if they went ahead and got all their shots completed before going into Knightsbridge.

Please advise.

Yuna, I *think* that vaccination worksheet is fine.

I had a tick missing for one of my vaccines listed, and when I queried it the grumpy older lady behind the reception desk did then tick it, and mumbled something about it not being needed. I guess that as the dates have been entered for a vaccination, the tick is irrelevant (though I'm not 100% sure).

The MMR doesn't need a code entered into the right hand box as it's complete. The codes are only used for waivers. The two dates show that both the initial MMR and the booster have been given.

Td is marked with a "B" waiver (Insufficient time to complete series) because it needs to be given every 10 years, therefore it can never be "complete". The date given within 10 years means it is valid for AOS.

Other than that ticks (which may/ may not be needed), I believe his vaccination worksheet is complete for AOS.

Most of the time they are ticked. I haven't actually ever seen one not ticked but should be okay. Notice how the tick boxes are only under the immunizations that have various options. I guess to indicate whether it was a DTP (baby shot) or a Td or a Tdap. Not sure why Chas got a tick for Tdap which he didn't receive.

Well thanks you two. You've never seen one? Eeeeek! I guess he's special then? I guess I will just hope that it's complete and correct, afterall him arguing with them a third time will do his stress level no good. Since they still refuse to check the bottom box, it's all up to the AOS officer anyway.

Piggy and Poo (?) went yesterday too and he messaged me that he argued big time about the check at the bottom. So much that she went and got the doctor to tell him they will NOT do that for non-immigrants.

I haven't seen one, but not that many post them. Pheebs sent me hers and It was ticked. I think all the old version forms I saw before this year have been ticked. You have dates so that's good. They just aren't consistent anymore at Knightsbridge.

Thank you for your email.

I have discussed this all with the Head nurse and Manager of the practice to confirm what we are saying. Due to the fact that it is a K1 visa, we are in fact unable to tick the ‘completed’ box. This is because the K1 visa is currently NOT an immigrant visa. Therefore under US Immigration regulations we are unable to tick that box for this type of visa.

I’m sorry that this is not what you want to hear, but this is by regulation of the US Immigration.

I don't know about other states, but TX doctors are real headache about getting vaccination records completed in the form ...First is almost impossible to find who just do the record part.. And they don't care about varicella history ticked in the form, because they want proof of immunity.. Kingsbridge gave me DTP shot which appears to be useless, because it doesn't have pertussis part .. And they blaming UK doctors for not knowing AOS vaccination requirements.. it's so frustrating..

Just a note to others. I requested my vaccinations back in August so I could try and get everything up to date. I have been waiting 3 months. My doctors surgery apparently has no GP until next month as the other doctors are locums. It must be signed off by a GP. We have one on retirement and the other is on maternity leave until next month then there is a huge back log. I may end up having to change doctors surgeries or getting all my vaccinations done from scratch. I dread trying to get our medical records printed for the move and so am filing the request early next year so I have A LOT of time to get it sorted.

Sometimes these things are not so straightforward and it's a good idea to get vaccinations done as soon as you can.
