And here is the FAQ sheet that comes with the vaccination worksheet.

The part that states:

"I am having my vaccinations elsewhere and am travelling to the US on a K Visa

If you have been given a copy of the vaccination worksheet, it means that you have chosen to defer the vaccines at this time. You must take proof that you have had the outstanding vaccines with the copy of the Vaccination Worksheet when you adjust your status in the US."

It states that I have "chosen to defer vaccines at this time" (I wasn't given a choice!), and that I must take "proof that you have had the outstanding vaccines with the copy of the Vaccination Worksheet".

So am I or am I not meeting all the vaccination requirements, because it implies that by being given a copy of the Vaccination Worksheet that I need proof of outstanding vaccines?

I have my medical tomorrow morning, hopefully I have all the paperwork I need (I'm missing my MMR and TD boosters from my vaccinations record but hoping to have them at the medical tomorrow.)

Is it only me that feels it shouldn't be up to the applicant/patient to make sure the paperwork is filled out correctly?

I think it seems like it's a bit of both. They should know how to fill it out but, because the individuals don't know the ins and outs of the process, how this can help with AOS etc. it seems to me that we have to put pressure on to get it filled out as much as they possibly can!


Yes, they offer them for a fee. The nurse there tends to tell K1s they don't need the shots and get them later. Insist that you want to be complete for US immigration right now so you don't have to pay another doctor in the US sign off on an immunization form.

But remember if you have one MMR and four weeks hasn't passed they can't give a second MMR. They should mark the waiver code. She may tell you to go get another MMR in a few weeks and may even write that it is due somewhere on your sheet, but she doesn't actually know USCIS and adjustment of status. Panel physicians are really only supposed to mark shots, dates, and waiver codes and not scribble their own notes about when you might be ready for another dose, but they tend to do that at Knightsbridge.

After my usual panicking, I delved further into the matter and noticed some faint text above the "Indicate reason below" far right column of the Vaccine Worksheet. They'd put a sticker with my DOB, AGE and PASSPORT NUMBER over the text, which as I wasn't familiar with the form meant that I couldn't see that the far right column is used for WAIVERS (when indicated with a code letter).

A quick Google Search located the Vaccination Worksheet document with the text visible in full.

That top box in the far right column should read:

To be requested if vaccination not medically appropriate.

Indicate reason below

A, B, C, D, F, H"

So if there is a letter in that column, it means it's a waiver for the vaccination on that row.

Not sure why I got an "Insufficient Time Interval to complete series" for the Td when I'd had the DTP (which contains (T)entanus and the higher strength (D)iphtheria) at the end of April 2015. They've waived it nonetheless, so hopefully that will get me through the AOS process. At least, that's how is appears to me now (he hopes!)

With regards to the vaccination worksheet, I noticed the following:

DT, DTP, DTaP - dated up to 1997. No date for when I had the DTP in April 2015 (no room for date?) Marked as "A" (Not age appropriate)

Td - Marked as "B" (Insufficient time interval). I had DTP done in April 2015, which contains TD.

Varicella Not Ticked but marked "D" - (Not routinely available)

Varicalla History ticked - (Indicated reason box left blank).

Posted below is a copy of the vaccination worksheet (minus any personal information/ details) for all to see.

In my opinion, you need a date 04-xx-2015 to get credit for that shot with USCIS. You got nothing in writing that says you were immunized for diptheria, tetanus, pertussis (or diptheria/tetanus) within the last 10 years. You were last immunized in 1997 according to your signed documentation. USCIS knows that is more than 10 years ago and should not grant an insufficient time interval waiver when it has been 18 years. Right?

It is a few posts up from when you started posting. I thought you already knew that stuff.

I guess I just had a bit of brain overload (I have read, read and re-read much of this forum and guides and probably try to take in more in one go than I should), plus trying to ensure I had remembered everything to take to the medical and also trying to remember everything to remember and bring up. Then when I got the Vaccine form (a photocopy) I didn't notice initially that the sticker was covering part of the text on the end column. Hence my panic.

Grrrr, so it sounds as if they've buggered it up. Yes, I totally agree, which is why I didn't get why they put a "B" for "insufficient time interval to complete series" Td waiver . My vaccination statement from my GP clearly stated tetanus and diphtheria (the equivalent of Td) were given on 28th April 2015, the same statement that showed my MMR was given on the same date (which they did enter).

I will be getting on to the phone to them tomorrow...

The fear of having to pay $600+ to a civic surgeon when we get to the States is causing panic over these vaccinations! I'm with you, ChasUno, I'm with you!

While I now feel more confident with what I'm supposed to do (largely in part to Nich-Nick - your posts really are mini goldmines) I'm still in a fret over it all.

Regarding your sheet, ChasUno, do we even think there's much chance of them changing it now?

SWJI89, yes, they can change the form if they've made a mistake, and as long as it's done before the medical is forwarded to the Embassy (which is done approx. 4 days after the date of the medical).

I've just got off the 'phone to Knightsbridge to discuss the matter. She agreed that they should have put in a date for the Td showing that I had received Tetanus and Diphtheria in the last 10 years (mine was done in April this year). She apologised for the omission and told me that they would email an amended copy of the vaccination worksheet later today. Hopefully once that's done then I can breathe a sigh of relief in the knowledge that my vaccinations are all up to date and meet US immigration requirements.

I also queried why the "Insufficient Time Interval to complete series" 'B' waiver was given for Td if it was up to date. She told me that all Td vaccines are marked with that waiver, whether they are given or not, due to the fact that as it is required every ten years, it can never be fully complete.

I guess the moral of my story is to be fully familiar with the vaccination worksheet, the dates that need to be on it (for each vaccine) and know the appropriate waivers required before attending the medical. And don't let them rush you, as the vaccination worksheet is the last part of the medical when you are likely to want to get out and on your way, and the 'nurse' (at least the one I saw) just seems to want to give your forms back to you and for you to go. When they give you your vaccination worksheet, go and take a seat opposite the booths and just run through it to be sure it has all been completed correctly.

Had my Medical this morning and despite now being full of holes, and my wallet being £330 lighter (I needed Td and MMR boosters) it all went pretty well!

At first they lost the letter from my doctor regarding a past bout of anxiety, but once that was sorted everything else seemed to go fine, and my vaccination sheet seems to be filled out correctly. I found all the staff there really friendly and helpful!

Special thanks to Nich-Nick, without whom I wouldn't have been as prepared as I was.

Knightsbridge doctors did indeed email me a copy of my amended vaccination worksheet as promised, in PDF format.

They also added one other entry to the form that I didn't ask them to do. This is despite me asking the interviewing 'nurse' (I actually wonder if she was just a receptionist as she had to go and ask another woman in the room behind her my questions) to confirm that I met all the vaccine requirements for immigration and AOS. She came back and said "yes", though as I subsequently found out upon getting home, I didn't as far as the original form was concerned.

The changes they made were as follows:

Putting a tick that was missing in the "DT, DTP, DTaP" box - I asked them to do this when they first gave me the form at the end of the medical.

Then in the amended form they sent back today:

They entered the date that was missing in the "Td" row (04282015) - this is what I asked them to do and now makes my vaccination worksheet complete for AOS.

They entered a 'D' into the waiver column for "Varicella History" (I didn't ask them to do this). The original copy form they gave to me had an entry that had been painted over with correction fluid/ Tippex. Not sure if it was entirely necessary as the crucial "Varicella History" had already been ticked.

In total there were three entries missing on the vaccination form that they first gave to me. One important missing entry would have made it invalid for AOS, the other two I'm not sure if they would cause any problems for AOS. Regardless, check and ensure that the vaccination form that they give back to you is correctly filled in. If you do it whilst you're at your medical, they can change it there and then. But you can also get them to change it over the phone and have it emailed to you. However, I believe any changes must be done before the medical results transfer to the Embassy (approx 4 days), after which it is too late to make changes and you will have to seek out a Civil Surgeon to sign off anything missing once you are in the USA.

If it helps others, here are copies of my completed vaccination worksheets to show what they should look like.

So... I just called my GP's surgery and asked what the best way to go about the shots there would be.

They tried initially to direct me to a travel clinic with the next appointment being in July (hah!). I told them that I needed to start sorting this out now ahead of getting to the medical and visa interview stages, to which they were sympathetic and have booked me in tomorrow to see a nurse to go through which vaccinations I will need. They said there will be a £10 admin fee to do so which will be payable tomorrow!

I'll let you know how I get on...

UPDATE: Went to see the nurse at my surgery. They found my MMR records. Turns out I had the MMR as a child and then had a Measles booster and then a Rubella booster but not the mumps! She gave me the mumps one today but it's now listed on my records as an MMR booster rather than just mumps. Hopefully that is enough to satisfy the requirements for that one?

I had a Pertussis jab as a child... so that means that I just needed Diphtheria and Tetanus, correct? I had the combined D and T booster today as well.

Hopefully that is me covered now, jab-wise!
