Doesn't matter as long as you capitalize things that are supposed to be, like names.

I would like to know if I checked the first box. should I check the second box too? or just one? I have already copies of my Federal tax returns for the pass three years.

If you will only send 2013, then only check the first box. If you will send all 3 years, you check BOTH boxes.

if I don't have anybody on B thro E. should I write 0 (zero) in the boxes or leave it blank?

I don't have it in front of me, but we filled the EZ and anything that had a box in that section we put a zero. There was just my husband (petitioner) and me (applicant) for a total of 2. So there was a 2 at the top and a two at the bottom. I left no box blank. To leave blanks gets checklists.

Definitely do not leave anything blank. Put zero. I did that and did not receive any checklists. Best wishes to u!

Not necessary but you can if your income indeed has cents.

on this Form 1040A. would my total income of that year be #15? and if so I should write it on #19 Federal income tax infromation. (Total Income) on the I-864EZ?

yes for tax form 1040 or 1040a it is total income you will put for the I-864 forms. If you file a 1040ez form it would be adjustable gross income
