I am planning to do mine myself . I will have my letter from my employer stating my job title pay rate and length of employment . I will have pay stubs going back a few weeks . Also W2's going back 2 years . Tax transcripts for 2011 and 2012 from the IRS . Bank letter stating my length of the account and current balance . I do not own a home or any other assets to put on it , so that will be blank for mine .

Read the instructions for the I-134 its helpful and any other questions many will help with them here . There is also a form I-864P that shows you the guide lines . If you feel you will not meet the poverty guide lines you can use a co sponsor to help meet the requirements of the I-134 .

Both you and your co-sponsor will fill out an I-134 and provide the supporting documentation for it.

Best Luck !

No. They can reside anywhere in the US.
