This might be asked before but I can't find the answer. I am wondering how one could get a copy of form DS 3025 for AOS when everything from the doctor is sealed? :bonk:

Usually the doctor/secretary knows you need that document for AOS and so, gives you a copy of it. Be sure they give it to you, or ask for it. Just as simple as that. :)

I had this same problem today, had everything ready for filing for the AOS apart from the copy of the ds-3025. I was never given one, even though it states on the bottom "Give copy to applicant" I didn't receive one at the doctors office or even think to ask, not knowing the role it would play in the future. Called the Consulate today and they told me that they sent two copies of it when they sent me my visa - they didn't, believe me I would have noticed. I saved and copied everything they sent me including the bag they sent it in. I do remember the lady at the Consulate telling me they'd mail me a copy during my interview, but I never thought to follow it up when it didn't arrive... I got lost in the excitement I guess. Anyway, I guess it was either forgotten or there was a mix up but they were very certain that they had sent it - I'm not going to argue with them, for obvious reasons, and they have always been really friendly and helpful whenever I had questions in the past and during my interview. They told me I now had two options, to call the doctors office where I had my medical and see if they had copies of the forms or to redo my vaccinations here in California.

I thought for sure I was doomed to get the MMR shot for the 3rd time in my life, as i didn't have much confidence that the consular doctor would still have the records.. however my wife told me to call and I did, they put me through to somebody who told me that they kept copies of all the records dealing with visa examinations, asked for my name then said they could copy them and asked how I would like them sent to me. Had my brother pick up the full medical results and forms a couple of hours later, scan them and email them to me. So I now have a copy of my signed ds-3025 and vaccination history and am able to send in my AOS package tomorrow.

I hope if you call your examining doctor that they have the records and are able to get them to you. Good luck!

That is odd. Tell them it is the vaccination worksheet, and that you need a copy of the vaccinations given at your medical for when you file for adjustment of status in the US.

As a last resort, you can ask the CBP officer that takes your sealed envelope when you arrive in the US to please make you a copy of your vaccination worksheet, DS-3025 because you really need it. They have made copies for people in the past. The worst that could happen is they say no and you need to see a civil surgeon in the US to get an I-693. Bring any vaccination records you have with you when you come to the US.

At least in Argentina, it seemed to be up to the doctor who did your medical. I wasn't given a copy of the DS 3025; some asked and were told they couldn't get one and the rest succeed in getting a copy of the form.... Now, there are 2 doctors available for medicals (instead of the 4 or 5 doctors there were before) and, at least one of them, gives you a copy without a problem.

Now I understand why it's odd. I was exempted for any vaccinations for applying K1 from Hong Kong, I was told that I will get my vaccinations in the US when I file for AOS. From looking at the list of shots that I need which I think I already have, I am now looking at how I can get my vaccination record in Hong Kong so that I don't need those vaccinations and they will give me a DS-3025 saying I have already had all required vaccinations!

Ask the interviewing officer at your consulate for a copy of the DS 3025 during your interview when you hand them the medical files, they should know why you need it and oblige.

I did ask the interviewing officer yesterday at the interview. They looked confused at first and later they even dug up someone else's DS-3025 to show me what it looks like and later found out that I will get mine when I apply for AOS in the states! :dance:

I do not think you will able to get it done now since you already had your medical. You will need to see a civil surgeon in the US and get an I-693 for AOS.

You need to see a civil surgeon and have Page 1 and the vaccinations section on Page 5 of the I-693 filled out. You can get the vaccinations elsewhere cheaper and bring the vaccination record with you when you see a civil surgeon. You are not required to get another medical, but a lot of civil surgeons insist on giving one and charging for it anyway. You may to need to call around and check out the post here on VJ about civil surgeons who don't rip you off.

The only vaccinations required for AOS are MMR, Tdap/TD, varicella(unless you had the chickenpox) and flu shot(only during flu season> Oct.-Mar.). You only need the first shot in a series to file for AOS.

I think it applies to USCIS not giving clear instructions on how to deal with K1 visa medicals, or doctors not following the instructions. That isn't limited to Hong Kong though.

Anyhow, too late to do anything about it in home country now. Please follow Jay-kay's recipe, amd congrats on upcoming wedding. :)

I-129F approved in 180 days from NOA1 date. (195 days from filing to NOA2 in hand)

Interview took 224 days from I-129F NOA1 date. (241 days from filing petition until visa in hand)

From filing I-129F petition until POE: 285 days
