2. G1145

3. Form I-485

4. copy of daniels passport

4. copy of his k1 visa

5. copy of noa2 approval

6. copy of valid i-94 front and back

7. copy of daniels birth certificate

8. g-325a

9. 2 passport photos of daniel

10. copy of vaccination documention DS-3025

11. i-864 and my transcripts from 2009 and 2010, and 1040 form from 2011 and my stock account information (as I'm close to the poverty level but a little short.

12. i-765.

can someone tell me where we mail this too exactly in chicago? gonna do the signature capture thing at the post office. and make sure it gets there. thanks

Yes, paystubs or a letter from your employer will suffice as proof of current employment.

You don't want to send in I-131 just in case?

Yeah, if you send the I-765, you need to make it separate (it's another application than the AOS one), and add 2 more pictures, and a copy of his passport bio-page... + another G-1145.

Yeah, if you send the I-765, you need to make it separate (it's another application than the AOS one), and add 2 more pictures, and a copy of his passport bio-page... + another G-1145.

This is not exactly true. You do need 2 extra photos, but the G-1145 is global, and the I-765 does not ask for a passport, so there is no need to include that evidence. Everyone should look at each application and give what it says in the instructions. The I-765 says it needs 2 photos.

That's fine, I have 4 photos of him and can add two with the I-765 how it states. I can put a cover letter before each different one, the I-485, I-864 and the I-765 if its needed.

Passport is not required, but you should submit something govt issued photo ID showing name and DoB if you have never been issued EAD before.

It doesn't ask specifically for a passport copy but it DOES asks for a government issued photo ID, and a passport bio-page copy is the easiest one.
