- Photocopy (Front and Back) of xxxx's Permanent Resident Card

- Photographs of xxxx and xxxxx

- Copy of Birth Certificate for Son, xxxx (Front and Back)

- Copy of 2009 federal tax form 1040 showing filing status as Married, Filing Jointly

I actually sent in over 20, spanning the 2 years, plus from our trip to Europe. I've seen here somewhere someone sent in like 5, I guess its up to you.

My check was cashed this morning - at least they are quick about getting paid. Now, we'll just wait for the NOA-1 and biometrics. We're leaving town for a week in about a month, so I'm hoping my wife's biometrics won't be scheduled then. I guess we'll just try a walk-in if they are.
