on the explaination that if the form DS-3025 from the embassey medical exam has an " incomplete " that we need to fill out form I-693 but the incomplete's that are on form DS-3025 are because of age related vacines that are not required for her to have Do we still need to fill out form I-693?

If she has all the vaccinations that are required for AOS for her age, then you do not need to get the I-693 done. Just send a copy of the DS-3025 with your AOS.

if the ds 3025 says incomplte, u need to have it transcribed by a civil surgeon in a I-693 form..thic link might help u in looking for civil surgeon who will not charge too much..

Have it transcribed over to the I-693 by CS part 1.2 and 5 only.

Try the Civil Surgeon locator on USCIS and maybe this link too: Civil surgeons who don't rip you off When you schedule your appointment, specifically tell the office of the CS that you are a K-1 applicant for AOS and that you need to have your Vaccination Documentation Worksheet, DS-3025, transcribed on form I-693, but ONLY Part 1(Info about you), Part 2 - VACCINATION CHART(some people understand you want them to transcribe Part 2 completely, which they can't do) and Part 5(the CS certification). Shouldn't cost more than $50. Mine was $40, but I also know people paying as little as $25. Good luck!
