i filed AOS and EAD without I-693 forms

hello again,

i filed AOS and EAD without I693 (report of medical exam) because i thought that complete copy of vaccination record is the only needed form. do i have to see a civil surgeon? thanks.

AOS

2/23/11 - POE (LAX) via k1 visa

3/20/11 - Married

4/27/11 - AOS and EAD packet sent to chicago lock box

5/2/11 - package received

6/4/11 - EAD, AOS receipt and ASC Biometrics appointment letter arrived.

6/29/11 - Biometrics done

7/1/11 - RFE issued

7/15/11 - RFE response received by DHS

9/12/11 - interview day!! whew

9/16/11 - received email notification that we passed AOS interview

9/22/11 received Green card! yess

ROC sept 16, 2013

31 March 2011 - Mailed thru USPS (AOS/EAD/AP)

03 April 2011 - NOA (AOS/EAD/AP)

18 April 2011 - successful walk-in biometrics

21 April 2011 - NOA (AOS transfer to CSC)

25 May 2011 - EAD/AP approved

04 June 2011 - EAD/AP combo card received

16 June 2011 - AOS/GC approved

22 June 2011 - GC received by mail

where can i find the "designated" civil surgeon?

the instruction for fiancee who had a medical examination within the past year as required to non-immigrant visa, you only need to submit vaccination supplement with form I-485.

i thought the only requirement for I485 is the vaccination record. :(

If you had had all required vaccination at the time of your medical overseas and your DS3025 is marked "complete" then you can just send that form with I-485. If you had to get any vaccines here in the US, then you have to have the vaccinations transcribed to I-693. You can find a list of designated Civil Surgeons on USCIS website.

It is over a year I did my medicals. Do I still need the surgeon to transcribe my rcords into the I-693 form?

If it has been more than a year since your medical exam, then you need to do a complete medical again at Civil Surgeon.

My Immigration Journey:

K1: June 2010 - December 2010

AOS: April 2011 - June 2011

ROC: April 2013 - August 2013

Naturalization: March 2014 - August 2014

If you had all your vaccinations and no others were needed you don't need the I-693 (also your medical must be less than a year old)

No you don't. OP is AOSing from a K1. As long as their medical is less than a year old and their vaccinations were completed they don't need an I-693

No. You need a completely new medical.

As stated, unless she is missing shots then her DS-3025 is fine. Some people have needed extra shots when they get to the US.

The I-485 actually needs a COMPLETE medical but the instructions state that as a K1 had their medical recently then as long as it's less than 12 months old they just need to be sure your shots are up-to-date.
