Vaccination Record Question

My husband got all his vaccinations in Scotland before he left but apparently did not get a copy of them to bring over here. He says that there was one in the packet that they took from him in the airport at his POE. But my questions is, what do we do now? Should he see if he can get a copy from his doctor in Scotland?

Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart, and Part 5, the Civil Surgeon's Certification, of Form I-693 (in an envelop sealed by the civil surgeon). Please see the instructions for Form I-693 for further information.

Or the inverse, if it was properly completed AND included as a part of the POE packet then it will be accepted

In my case I had all the vaccines, but no records. I needed to contact my doctor in Brazil, which is my Dad and ask him to make a certificate showing that I took all the vaccines. The Clinic told me that the certificate doesn't need to be original, they just need the stamp and signature, and to send via fax.

I think it would be better to ask your immigration doctor how he wants the records, then you contact the one in Scotland.

Some of the advice you've received here is WRONG but also so partially correct it will cause you issues.

You HAVE to include a copy of your immunisations, whether it be the DS-3025 or the I-693... Remember that you need to show proof you comply with the immunisation stuff NOW, at the time of filing AOS, the record they currently have is for the K1, so it might NOT be current for the AOS. Some shots require several shots to complete the series so you need to be sure he is current NOW.

If your shots are COMPLETED then you don't need to do a I-693, but if they're not completed then you will need to do the vaccination section of the I-693. At the very least you will need to include a copy of the DS-3025. I believe I read one person didn't and was okay, but the majority will get an RFE. To be safe, I would request the Scottish doc to fax or email you a copy. Or get a family member/friend to pick it up for you and scan and email it to you. It's the only way to be SURE you will be okay and it will only take a phonecall to find out if a copy is available for you to get (it says on the bottom of the form he's supposed to have a copy, but thy might not have kept a copy on file themselves).
