Do I send both DS-3025 AND I-693?

After a week of running around trying to find a civil surgeon that didn't want to make me retake the entire medical, I now have my vaccinations transcribed from my DS-3025 onto the I-693. The envelope is sealed and ready to be sent off with the rest of my AoS application.

My question is this:

I know I need to submit the I-693 with my AoS application, but do I also need to include my original DS-3025 vaccination sheet (the one I was given at my k1 visa medical)?

The information on my original DS-3025 is no longer up-to-date, because it says that I need a teatenus booster. After completing my medical last November, I had my booster done at my family doctor and gave the civil surgeon the evidence to prove so. He transcribed the information from my DS-3025 onto the I-694, updated the vaccine chart to indicate that I had my vaccine. So my thinking is that technically, the DS-3025 is worthless now?

But do the USCIS want to see it anyways? I am worried that because the information on it conflicts with the I-693 information, the USCIS might miss some detail and send an RFE.

Should I just send both of them?

I really wish I had just gotten that teatenus booster done at my k1 medical. The nurse seemed so certain that this wouldn't be an issue, but it's been nothing but stress and worry!

don't worry about it so much. I sent a copy of my DS-3025 and a copy of the I-693, but I think you should put on the cover letter that you had the medical done during the K-1 visa thus only the vaccination part is filled out.

When I did my AOS they sent me an RFE coz they said the I-693 wasn't filled out completely. But it was coz I didn't get another medical, which K-1 shouldn't and I guess people doing the AOS don't know that lol

Anyway, don't be so stressed about it :) Just send them the copies :)

I personally sent both. The i-693 overrides the DS-3025 (for vaccinations etc) but I sent the DS-3025 to prove the date i had the K1 medical and that I HAD a K1 medical. I think you should send both just to be safe but I haven't read in the requirements that you NEED to.. I just think it's safer.

Don't send the original but send both. They are smart enough to figure out different dates and all that.

I wouldn't send both. People send the DS3025 instead of the I-693 when they are sure they had all the required shots and don't want to spend a week tracking down a civil surgeon. You needed an additional shot recorded, so the I-693 was unavoidable for you.

If the USCIS contractors correctly order your A-file when they go through your application, then all will be good. If anything, you might want to mention in a note attached to the I-693 that you have had a medical exam within the one-year time frame that they will find in your A-file.
